Grant Application
The following grant guidelines reflect Healthcare Foundation criteria for the acceptance and evaluation of grant requests. All applications must provide information as requested by the criteria.
All submissions will be reviewed by Healthcare Foundation’s Grant Review Committee and approved by the Foundation’s Board of Directors. The Committee shall assure that all requests for grants are consistent with the mission/vision of the Foundation and adhere to Foundation guidelines.

Grant Guidelines
Eligibility Criteria
- Program, Service or Capital Improvement directly enhancing healthcare system, service or delivery within our District
- Exhibits values consistent with the goals and mission of the Foundation
- Proposal substantiates the need, demonstrates future sustainability and measurable outcomes
Non-Eligible Requests
- Loan Request
- Budget Shortfalls
- Ongoing Operational Expense
- Salaried positions (aside from start up or defined duration positions)
Priority of Grant Request
- Addresses Critical Need
- Highest Impact on Healthcare within our District
- Seed money or investment to lead to independent funding or sustainability
- Match donor intent or specific Foundation priorities
Review Grant Request
- Meets Eligibility Requirements
- Determines the Priority of Grant
- Confirms the Foundation is the best source of funding
- Assesses credibility and history of requester in meeting grant requirements. (Timely reporting of outcomes, and financial management)
Process Grant Request
- Coordinates with Finance Committee to confirm availability of funds
- Establishes Grant Reporting Requirements (measurable outcomes, progress reports)
- Advances recommendations (approved or denied) to the Board
Monitor Progress
- Review progress reports for grant compliance
- Request correction, completion of incomplete and delinquent reports
- Upon completion of the grant, prepare a report on measurable outcomes / impact on district healthcare for the Board
